1. Scope of Services
Sleep Coach MD provides virtual consultations, home sleep testing, and personalized sleep treatment plans. Services are available for individuals 4 years of age and older in Texas and Florida.
Our services do not replace emergency care or general medical care. If you experience a medical emergency, call 911 or go to your nearest emergency room.
2. Eligibility
By using our services, you confirm that you:
- Are at least 4 years and above
- Reside in Texas or Florida without regard to its conflict of law principles.
- Are capable of entering into a binding legal agreement
- Will provide accurate and complete information
3. User Responsibilities
You agree to:
- Provide true and complete medical history and personal information
- Use a secure internet connection for virtual visits
- Keep your login information confidential
- Inform us promptly of any changes in your health or contact details
4. Cancellations and Refunds
Refunds for services already rendered are not available.
We value your time and ours. When you book an appointment, that time is reserved exclusively for you.
a. Payment Policy
All appointments must be paid in full at the time of booking.
Payments are non-refundable except as outlined below.
b. Cancellations & Rescheduling
Cancellations made more than 48 hours before your appointment may be rescheduled at no additional charge and the appointment must occur within 3 months of the cancellation.
Cancellations made less than 48 hours before your appointment will not be refunded.
No-shows (failure to log in at your appointment time) will result in the full fee being forfeited.
c. Exceptions
We understand emergencies happen. In rare, documented cases such as hospitalization or a verified urgent circumstance, we may, at our discretion, offer a one-time courtesy rescheduling, and the appointment must occur within 3 months of the cancellation.
d. Technical Issues
If a technical failure on our side prevents the appointment from taking place, we will reschedule at no cost.
e. How to Cancel or Reschedule
Please notify us by email or our secure patient portal as soon as possible if you need to cancel or reschedule.
Why This Policy Exists
Last-minute cancellations mean we cannot offer the slot to another patient in need of care. This policy helps us ensure fair access to timely appointments and maintain the quality of service for all patients.
5. Telehealth Risks and Responsibilities
By using Sleep Coach MD’s telehealth services, you acknowledge the potential risks, including:
- Incomplete data due to technology failure
- Limited physical examination
- Delays in diagnosis or treatment
- Security breaches despite encryption measures
You agree to participate actively and responsibly in your telehealth care and understand these limitations.
6. Intellectual Property
All content on our website, including text, logos, and images, is the property of Sleep Coach MD and may not be reproduced, distributed, or used without permission.
7. Limitation of Liability
To the fullest extent permitted by law, Sleep Coach MD is not liable for indirect, incidental, or consequential damages arising from the use of our services. Our total liability will not exceed the amount paid for services in question.
8. Termination
We reserve the right to terminate or suspend access to services for violations of these terms or for conduct that harms others or disrupts our operations.
9. Governing Law
These Terms are governed by the laws of the State of Texas and Florida, without regard to its conflict of law principles.
